PSLF Fax Number: Where to Fax Your PSLF Form (2026)

Most "PSLF fax number" lists online are out of date. The Public Service Loan Forgiveness form no longer goes to your loan servicer's general fax line — the current form routes straight to the U.S. Department of Education, and the fastest path skips faxing entirely. Here is exactly where each document goes.

!

Check the form before you send. Federal student aid submission routes have changed more than once in recent years, and this page — although verified against official sources — may lag behind or contain errors. Section 8 ("Where to Send This Form") of the PSLF form you downloaded, and StudentAid.gov, are the final word. FaxFlow is not affiliated with the U.S. Department of Education, Federal Student Aid, or MOHELA.

Which Fax Number Do You Actually Need?

A. You are submitting the PSLF form itself

The PSLF & TEPSLF Certification & Application (OMB No. 1845-0110) is processed by the U.S. Department of Education. Section 8 of the current form, "Where to Send This Form," gives three routes:

Fax

540-212-2415

Mail

U.S. Department of Education
P.O. Box 300010
Greenville, TX 75403

Upload

Log in to StudentAid.gov and visit My Activity

B. You are sending other documents to MOHELA (your servicer)

If your federal loans are serviced by MOHELA and you need to send servicing paperwork — forbearance or deferment forms, correspondence, proof documents — MOHELA's contact page lists a general "Send a Letter or Submit Documents" channel:

Fax

866-222-7060

Mail (documents, not payments)

MOHELA
633 Spirit Drive
Chesterfield, MO 63005-1243

MOHELA's page adds: "For fastest processing of documents, upload via your online account" — and pointedly notes that "The PSLF program is managed by the U.S. Department of Education, not MOHELA." So do not send the PSLF form here expecting PSLF processing.

Honest Advice: Try the PSLF Help Tool Before You Fax

The form's own instructions describe the preferred path. The PSLF Help Tool at StudentAid.gov/pslf searches the PSLF Employer Database, generates the form pre-populated, and lets you "request the Authorized Official to sign this form electronically. By signing electronically, this form can be submitted for review without the need to print it out." No printer, no fax, no mail.

Faxing earns its keep when the digital path breaks down: your employer's authorized official will not sign electronically, the organization no longer exists, or you already have a wet-ink-signed paper form in hand and just need it delivered with proof.

Faxing the PSLF Form: Three Rules That Trip People Up

  1. 1. Fax only the form pages — omit pages 4–16

The instructions printed on the form say: "Please omit pages 4–16 when mailing or faxing back." Pages 4–16 are instructions and definitions. Sending the whole 16-page PDF slows intake and burns fax pages for nothing.

  2. 2. Use an acceptable signature

A manually submitted form must carry one of these:

    • a handwritten signature in dark ink,
    • a hand-drawn electronic signature made with a mouse or finger on a device, or
    • a digitized image of a handwritten signature embedded on the signature line.

Typed names and certificate-based signatures are not accepted — "even if using a font that mimics cursive text." Both your signature and your employer's must qualify, or the form bounces.

  3. 3. Certify on a schedule, not just at the end

The form advises completing it "annually or any time you change employers or have a change" in employment status. Certifying as you go keeps your qualifying payment count current — far easier than hunting down signatures from an employer you left years ago.

Sending the Fax: A 2-Minute Checklist

  • Pages 1–3 only, fully completed, with acceptable signatures from you and your employer.
  • A cover sheet with your name, the last four digits of your SSN or account number, a page count, and "PSLF form" as the subject.
  • Fax to 540-212-2415 (or the number printed in Section 8 of your form, if it differs — the form wins).
  • Keep the transmission confirmation. FaxFlow stores a timestamped delivery receipt for every fax — useful if your payment count is ever disputed.

PSLF Fax Number FAQ

What is the fax number for the PSLF form?+

Section 8 of the current PSLF & TEPSLF Certification & Application (OMB No. 1845-0110) says to fax the completed form to 540-212-2415, mail it to U.S. Department of Education, P.O. Box 300010, Greenville, TX 75403, or upload it by logging in to StudentAid.gov and visiting My Activity. Always follow Section 8 of the form version you are actually submitting.

Do I fax my PSLF form to MOHELA?+

No — the current PSLF form routes to the U.S. Department of Education (fax 540-212-2415), and MOHELA's own contact page notes the PSLF program is managed by the Department of Education, not MOHELA. MOHELA's fax, 866-222-7060, is its general "Send a Letter or Submit Documents" line for other servicing paperwork on MOHELA-serviced loans.

Which pages of the PSLF form do I fax?+

Only the completed form pages — the official instructions say "Please omit pages 4-16 when mailing or faxing back." Those later pages are instructions and definitions; faxing all 16 pages just slows processing.

Is there a faster way than faxing the PSLF form?+

Usually, yes. The PSLF Help Tool at StudentAid.gov/pslf generates the form, lets you ask your employer’s authorized official to sign it electronically, and submits it for review "without the need to print it out" — no fax or mail at all. Faxing matters when your employer will not sign electronically or you are submitting a manually signed paper form.

What signature does a faxed PSLF form need?+

An acceptable signature is a handwritten signature in dark ink, a hand-drawn electronic signature (mouse or finger), or a digitized image of a handwritten signature embedded on the signature line. Typed names and certificate-based signatures are not accepted — even in a cursive font — and will get the form rejected.

How often should I submit a PSLF form?+

The form instructions recommend completing it annually, or any time you change employers or have a change in employment status. Regular certification keeps your qualifying payment count current instead of reconstructing years of employment at forgiveness time.
