What Is Form 433-D?
Form 433-D, "Installment Agreement," is the document that records the terms of an approved IRS payment plan. The IRS typically sends Form 433-D to you once your installment agreement is approved so you can confirm the monthly amount, the payment date, and — if you choose — authorize direct debit from your bank account. It is different from Form 9465, which is the request you submit to ask for a plan.
