Purpose

Used to file appeals of NY Unemployment Insurance decisions with the Appeal Board (faxed or mailed within 20 days of the ALJ decision) and to request hearing transcripts after appealing. Appeals require a written request including the Administrative Law Judge case number; transcripts can be requested in writing (within seven days of the Notice of Receipt of Appeal) with a deposit for the transcript, mailed to the specified addresses, or faxed.

Appeals must be postmarked or faxed within 20 days of the decision date printed on the ALJ decision. Include the ALJ case number in the appeal.
