Medical Protection requires a doctor/medical professional's letter on letterhead stating the illness and the household member's name and address; Infant Protection requires a valid birth certificate or equivalent official documents; If your account is not on a Discount Rate, or you are unsure, fill out the Financial Hardship Form on the back of this application.
