Submit a written extension request via fax to PennDOT; include the reason and any rejection correspondence; upon authorization, PennDOT provides an authorization number to place on the new form MV-1/MV-4ST/MV-120. The agent then issues a new Temporary Permit and must fax a copy of the new form with the authorization number to PennDOT within 48 hours to finalize the extension.
