Use Judicial Council Form DE-172 to file a creditor's claim in probate. Include an itemized claim, indicate the basis of the claim, attach supporting documents, indicate whether secured, and provide details on any security. File with the probate court and mail a copy to the decedent's personal representative. The deadline is the later of four months after letters were first issued or sixty days after notice of administration. A proof of mailing or personal delivery must be completed.
